Give office at least 1 week notice to change or move an appointment and reschedule to come in within 2 weeks. Any appointments not used within the contract period will be forfeited without refund. Does not apply to new patient exam, cleaning, and x-rays, or same day cleanings. After hours emergencies not covered. Cannot be combined with any other offers. Additional exclusions apply. Not available with CareCredit transactions.
